造血幹細胞移植とトロンボモジュリン製剤（リコモジュリン）

トロンボモジュリン製剤（rTM：リコモジュリン）は、その抗凝固活性のため播種性血管内凝固症候群（DIC）治療薬として世界で唯一日本でのみ認可されている優れた薬物です。

本記事執筆時点での保険適応はDICのみですが、抗炎症効果（Abeyama K, et al. J Clin Invest. 2005; 115: 1267-74）やその他の作用もあり、多くの病態に対して有効ではないかと期待されています。

急性前骨髄球性白血病（APL）に対して、rTMが、ATRAによる抗線溶作用（N Engl J Med. 1999; 340: 994-1004.）や抗白血病作用を増強するという研究結果は、APLに合併したDICに対してrTMが良い適応であることを示しており、大変に興味深いところです。

 rTMが、急性骨髄単球性白血病細胞株であるTHP-1を分化させて細胞増殖を抑制したという報告もみられます（JNK/c-Junシグナルを活性化することによる）。

造血幹細胞移植後に合併した、類洞閉塞症候群（sinusoidal obstructive syndrome:SOS）、血栓性微小血管障害症（thrombotic microabgiopathy：TMA）、生着症候群（engraftment syndrome）、GVHDに対してrTMが有効であるという報告もみられています。
